Registered Nurse Senior Navigator - Full Time

Required Education and Experience: Minimum of 2 years oncology nursing or surgical experience, either inpatient or outpatient. Must obtain Nurse Navigator certification within 2 years of employment.

Preferred Education and Experience: Preferred experience in patient education, knowledge of continuum of cancer including types of cancer, causes, screening and diagnosis, treatments, and support services.

Additional Eligibility Qualifications: Excellent verbal and written communications with internal partners and external partners. Need to be sympathetic and provide emotional support to patient and caregiver(s), multi-task. CPR certification within 3 months of employment.

Summary/Objective: A medical professional whose clinical expertise and training guides patients and their caregivers to make informed decisions, collaborating with a multidisciplinary team to allow for timely cancer screening, diagnosis, treatment, and increased supportive care across the cancer continuum.

Coordinates and schedules surgeries for all surgical facilities. Keeps physician apprised of changes in the schedule.

Demonstrates awareness of self-responsibility and accountability for own professional practice. Practices nursing skills in accordance with licensure and Clinic P + P. Exercises good judgment in decision making to ensure quality patient care outcomes. Acts as resource person to Clinic and outside caregivers.


Essential Functions

Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance to the cancer patient from screening to survivorship. Systematically and continually performs the functions of assessing, planning, implementing and evaluating the care according to the nursing process and physician direction.

Provides education and information to the patient and family, helping to make the care seamless, continuous and comprehensive. Initiates and documents patient teaching including family and significant others based on assessment of needs. Responds to patient request for information regarding the disease process, expected side effects of treatment, and community resources. Uses appropriate patient education documentation tools.

Supports the patient during difficult decision-making periods.

Functions in an organized and time conscious manner. Partners with patients, families, interdisciplinary team, and community resources to provide well-coordinated, timely compassionate, exemplary, interdisciplinary care. Communicates with all members of the healthcare team, as appropriate, about patient/family needs and concerns.

Initiates and performs ongoing review of policies related to service provided. Identify and help implement improvement processes. Where appropriate, updates or writes new polices to enhance professional practice
